There is the simple way of using a cord to hold a gemstone donut (see first link below). And then there is the macrame way which really dresses up the whole design.
These free macrame tutorials are by Tanja Zivanovic of Macrame Magic Knots. Both tutorials involve making a long macrame section which is then doubled to hold the donut. She is meticulous in removing all the excess cord ends before she begins the necklace portion.
Also notable is the novel way she makes a sliding knot to fasten what is an adjustable necklace. Clever!

The second one is gorgeous but more challenging.
